Freshman applicants who have successfully passed medical examinations are requested to submit all the entrance requirements to the Office of Admissions on April 14 & 15, 2011 for pre-enrollment.

Admission Requirements:

• High School Report Card (original) or Original Transcript of Records & Honorable Dismissal (for Transferees)

• Certificate of Good Moral Character from High School Principal/Guidance Counselor (original)

• Photocopy of Birth Certificate
